Are you living with heart failure?

We invite you to participate in an online survey, “Perspectives from Patients with Heart Failure.”

Global Heart Hub, in collaboration with Illinois State University, is conducting a patient-centric research study created by patients for patients. The purpose of this study is to examine perspectives of patients diagnosed with heart failure to guide future development of educational materials.  

This survey is for anyone who is 18 years of age or older and identify as an individual who has been told they have heart failure. The participation in this study is voluntary. Participants will not be penalized for choosing to skip parts of the survey, not participate, or withdraw from the study at any time.   

Study responses are anonymous; nothing that will identify participant will be linked to the responses. The findings from this study may be presented in future professional meetings or published in a professional journal. Findings will only be presented in a collective or aggregate manner.  

The survey contains questions based on heart failure perspectives and will take approximately 5-10 minutes to complete.
